Red face 96 Diesel Engine Valve and Piston Break

I am new to the site and interested in members thoughts on why a 96 E300 diesel engine would all of a sudden have a valve break and destroy some pistons. I have had the car serviced at the same German specialty service company for 10 years. I had a head gasket repair done 9 months ago which I understand is fairly rare. The mileage was low at 165,000 miles. I keep seeing where owners have seen these engines go twice as long. I have used Valvoline synthetic oil for 13 years.

Could there have been an error in servicing? Has anyone one else experienced this problem? I have decided not to repair the engine due to the costs although the car is in good condition otherwise. I purchased a 2005 320 cdi with 91,000 miles and hoping for a positive future. I have driven MB diesels since 1980.

Your thoughts are appreciated.
