Default 91 350SD - Engine shake at idle +

I have a 91 350SD. Bought in 2001. In 2002 the crank-shaft went thru the engine body, oil leaked out and engine froze dead! The dealer rebuilt the engine under used car warranty, but the engine has never been the same ...

Symptoms: The engine shakes at idle. The exhaust is very smokey for at least 5 minutes after starting. The shake is rhythmic ... almost as if engine mounts are bad/worn!? The idle was increased by a local shop but that doesn't seem to help the shaking. Overall, the acceleration response is slower than before. The shake becomes more pronounced when it's in drive (as opposed to Park).

I have changed the fuel filter and air filter, but this hasn't improved things.

What other things should be checked out? Any advice would be much appreciated. Also, if you are a repair shop specializing in Mercedes Diesels (in NJ), please post your contact information here.
btw. I live in central NJ 07095.

I have a 90 sdl....the idle is very smooth.
besides worn mounts I would guess shaking is caused by one or more cylinders not firing correctly.
A compression test will tell the story on the cylinder/head/valve side.
Checking the injectors would settle the feed side.
