BAM!! hole in my engine 97 E320 !!

Default BAM!! hole in my engine 97 E320 !!

something caused my engine to totally blow up. driving along, it began to miss and chug, nearly dying at low rpms. keeping it alive by accelerating enough to keep rpms at about 3000. made a detour and went straight to the stealership. on the way, it went BAM. got out of the car and oil was pouring out of the engine with parts of the engine on the asphalt. there was a hole the size of a half dollar blown in the engine. fast forward.... after replacing the motor with two motors, the car exhibits the same symptoms it did before blowing up. the mechanics at the stealership THINK its the cam adjuster, but the cam adjuster has been different three times now, with the same symptoms present each time. so, how can that be??? three bad cam adjusters?? highly unlikely. the problem ended up being that the #6 piston collided with the exhaust manifold. the timing was off - to say the least. what would cause this????? no one here knows. what would cause the timing to be so off on this car? would love to get ideas.

Piston colliding with the exhaust manifold?
What happened to the head, that suppose to be between them?

didn't replace the cylinder head? they did pressure test on it?

a small crack or pin hole on the cylinder head could cause the coolant to get in the piston and bent it.

got any photo of the broken engine? can you post it? we need to analyse the pattern on how it breaks.
