Default Front Brake Rotors

I have a 98 E320 and need to replace my front pads and rotors. The front calipers look pretty straight forward but the rotors are another matter. After removing the hex set screw from the rotor, I cannot determine where the rotor breaks away from the hub. Their is a cap that covers the hub but I don't know if you have to remove it. If so, does it screw off or require a special wrench. Most rotors I have replaced you can bump off the rotor once your remove the caliper. Any help hints would be helpful.

yes remove caliper and then the rotor screw, then hit the rotor with a large hammer

Default suggested rotor and pads

I noticed that my rotors are pretty scored and have an edge on the outside and inside track of the rotor. Not sure if that is normal. Do you suggest replacing both pads and rotors at the same time rather than turning the rotors? If so, can you point me to a good priced source for pads and rotors.
