Default Misfire Code P0304 and Ignition Coil Code P0354

Hi

I am getting a misfire code cylinder 4 and open circuit code for coil D. It is intermittent but occurs more than it does not occur.

I believe the true error is P0354 and P0304 is a consequence of this fault.

For prelim test I swapped the plug and ignition coil for Cycl 1 - the problem did not transfer. So plug and coil appear ok, so eliminate P0304.

To me it appears that the signal controlling the coil firing is failing (P0354). Where does this cable come from within the engine (location pic would be awesome).

Is the primary controller a discrete unit for all cylinders or is there an individual controller for each cylinder?

Thanks

EDIT:
I am not sure if will be relevant but I then also get codes P0016 and P0017.
