e 400 tranny

e 400 tranny
in a totally unpredictable and intermittant fashion, my tranny has slipped upon being put in d. Reves/slips then does or does not engage. has only happened twice, weeks apart. Just happened on small grade by my house, seems to occur on startup or shortly after. Any thoughts/ suggestions,
