Default Grinding noise- Transmission?

Hi guys its fellbie again and i am writing to inquire about something that was happening to me for a longer time but only now i decided to gather some info.

As you can see in my profile i have got a 2007 G 320 CDI. Since about a year or so i can hear a sort of griding noise if i open my side windows. The noise occurs mostly after the transmission shifts, it can be heard for about 1 second, then it disappears. Sometimes it can be heard when cruising and i apply a gentle touch of throttle ( about 15% ). When i got uphills usually when the tranny shifts into 3rd gear i get the same old grinding noise but the the car does not really transmit power and it jerks for a bit. However, the cause for me to finally want to find out whats happening is that approximately every week or so after moving from a red light when the car shifts into 2nd or 3rd ( not so sure ), when i have my foot on the throttle suddenly it happens that the revs climb for about a second and only then the next gear gets selected. The car has no fault codes. Any idea what could cause this problem? Because the noise is not a very nice one, it doesnt sound really healthy. It is not loud and the car drives for a long time with it but these additive symptoms are scaring me very much, i am just hoping that it isnt a total transmission failure, requiring a swap.

Thank you very much for any potential answer
