1993 E300D Transmission Problems

Im having a problem with my transmission when the transmission is cold it shifts really fine.
after its warm and I start driving again it slams hard into the next gear.
my commute is 20 miles. where I work is seconds from the highway. I will do 70 miles an hour for 19 miles with no problem. when I get off the highway from a dead stop at the light it will shift hard it will high idle then slam into the next gear
can someone enlighten me on what could be the problem?
if it helps any after I did the brakes on the car that's when the problem started. I can answer back very quickly as the notification email goes to my phone

124 transmission

Any slipage or hesitation going into reverse is a sign of a bad transmission. Also the front and back ends of the drive shaft have a rubberized flexible drive couple that is a maintenance item. Having one of these fail is a very bad thing. Probably around a 200km-250km lifespan on transmissions, depending on a lot of things.
